Week 5

Seeing as how I have returned from my journey and fixed my decimated sleep cycle, the work can continue!
I began this week by talking to Dr. Ajmani-White about how to phrase my questions. The key information that I want to gain is: whether or not the person has a phobia, how severe it is, what aspect of the trigger scares the person, if there is a certain event that could have sparked it, and how much it affects daily life. While I could ask for more information, I want to keep the survey as brief as possible as a means of not deterring potential surveyors. This is what I have so far:
This survey consists of 6 questions relating to phobias. A phobia is defined as an intense fear or anxiety about a specific object or situation.
Question 1: Are you afraid of anything that fits the description of a phobia, if yes can you describe what that is, if no then the survey is over!
Question 2: Is there a specific aspect of this thing that scares you?
Question 3: Can you pinpoint a situation in your life that could have sparked this phobia?
Question 4: How intense is this fear on a scale of 1-10?
Question 5: How often do you think about this phobia? (Never/Rarely/Occasionally/Often/Daily)
Question 6: (optional) What are your age and gender?
One of the main flaws I can see occurring is that a good portion of people will simply answer no to the first question, leaving the rest of the survey pointless. Nonetheless, I have been looking into ways to disperse my survey to as many places as possible.
Outside of everyday office things like shredding papers and changing air filters, I have been working on compiling a list of affordable places to get psychologically tested. Additionally, I have included the link to my completed survey so anyone can fill it out:
